Position Information:Department: NursingWork Arrangement: Full OnsiteShift: Sunday, Wednesday, FridayPrimary Responsibilities:Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessaryProvides report to nurse coming on duty, including sufficient information for follow-up action if necessaryConducts resident rounds, reporting problems to nursing supervisor and initiating corrective actionAdministers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations dailyPerforms comprehensive assessments of each resident on unitReviews and approves admission and re-admission nursing assessments completed by an LPNDevelops care plan based on each resident's assessmentObtains physician orders and any revisions for all treatment and medications neededAssesses on a weekly basis the skin condition, stage, size, site, depth, color, drainage and odor of conditionRenders immediate assistance to any resident in needSupervise and evaluate all direct resident care and initiates corrective action as necessaryProvides clinical supervision of nursing assistants providing direct patient careAssumes responsibility for compliance with federal, state and local regulations within the assigned unitCounsels nursing assistants and communicates clearly with Clinical Nurse Managers and/or Director of Nursing if issues ariCollaborates with interdisciplinary team to assure that high quality resident care is maintainedPrepares nurse assistant assignments and explains all pertinent resident careSupervises distribution of nourishmentsSupervises the removal of all postural supportsOrders, receives and documents medication according to facility procedureSupervises response to residents calls for assistance and responds to call lights as necessaryInsures that all physician renewal orders and physician documentation is timely and appropriatePrerequisites:Graduate of an accredited Registered Nurse school of nursingCurrent and active RN license in the state of OregonCurrent certification in Basic Cardiac Life Support in accordance with AHS or ARC standardsKnowledge of Geriatric Nursing requiredBenefits with CareNational:Health, Dental & Vision InsuranceWeekly PayReferral Bonus401(k) MatchingIf you're passionate about providing high-quality care, apply today, and one of our recruiters will reach out to you immediately!#LI-NATIONAL
