Adult Community Clinical Services (ACCS) provides clinically focused evidenced based interventions to adults with severe and persistent mental illness. Services are person centered and are delivered by a multidisciplinary team of professionals in either residential or community based settings. The ideal candidate possesses knowledge and experience with mental health, nutrition and medical care. Strong interpersonal skills that allow for sustained client engagement and health participation are crucial for the development of a trusting relationship.Responsibilities:Work closely multidisciplinary team to provide education related to medical health, medications, and other related topicsDevelop and conduct training for staff and clients related to health and wellness topics, including but not limited to nutrition and physical activityAssist with identifying and accessing community resources that support client needs and wellness effortsGuide and support clients in understanding their medical conditionsMeet with clients and staff to discuss medical diagnoses, including causes, symptoms, complications, treatment options and preventionProvide direct medical care for clients including monitoring vitals, and wound care as neededReview all medical documentation for assigned clientsAssist both clients and staff in assessing symptoms and determining appropriate responses/actionsQualifications:Requires an associates degree in nursingLPN or RN nursing licenseValid Massachusetts drivers licenseMust possess a current RN license from the Commonwealth of MassachusettsSchedule: Wednesday-Saturday, 9am-2pmHourly Rate$37,340—$37,340 USDAt Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
