Job Title: Transition Coordinator Company Overview: Exodus PCS is a rapidly expanding and highly regarded provider specializing in Relocation Service Coordination and the Moving Home Minnesota programs. We are dedicated to assisting nursing facilities and residents in Substance Use Disorder (SUD) treatment to find suitable community housing, including Assisted Living, Foster Care, and Independent Living options. Our goal is to ensure a seamless transition for residents by coordinating with providers and community resources, ultimately fostering life-changing opportunities. Position Summary: We are seeking a full-time Transition Coordinator to join our team. The two main roles of this position are coordinating and assisting with a clients belongings for discharge and being a case manager with an active caseload. This role offers the flexibility of hybrid work (approx 50% remote, 50% onsite). The successful candidate will be self-motivated, dependable, and compassionate, with a strong background in health and human services. Key Responsibilities: - Facilitate the transition of residents to community housing by coordinating with relevant case managers, providers and community resources. - Collaborate, create and execute plans for client belongings including assisting with packing, sorting, moving and coordination of necessary items. - Manage case plans, including medical assistance, housing coordination, and discharge planning. - Utilize knowledge of waiver programs and community resources to support residents effectively. - Conduct remote and onsite work with travel throughout the Twin Cities Metro area. - Complete required documentation and maintain case records in compliance with organizational standards. Qualifications: - Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services, Nursing, or a related field. - Minimum of two years of experience in human services, social services, or healthcare. - Proficient in community resources, waiver programs, and county applications. - Experience in medical assistance, housing coordination, case management, and related areas. - Must pass a background check conducted by the Department of Human Services. - Possess a reliable vehicle with insurance and a valid driver’s license; mileage reimbursement is provided at the federal rate. - Proficient with Microsoft 365 and Microsoft Teams. - Ability to lift up to 25 lbs.
