divh2Nursing Assistant/h2pThe purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with the facility policies and procedures and report resident needs and concerns to a licensed nurse./ph3Education / Knowledge/h3ulliMust be a graduate of an approved Nursing Assistant Training Program, or be eligible to enter such a program, graduate and pass a state approved competency test (written skills) within 120 days from the date of employment./liliPrior experience as a care provider is preferred./liliMust possess the ability to make independent decisions when circumstances warrant./liliMust possess the ability to deal effectively with personnel, residents, family members, visitors, government agencies and the general public under all conditions./liliMust be willing to follow directions./liliMust be dependable in attendance and available by telephone and/or message phone number on reasonable notice./li/ulh3Delegation Of Authority/h3pAuthority is delegated to the individual in this position to:/pulliProvide care to residents following facility policies and procedures/liliReport residents needs and concerns to a licensed nurse/li/ulh3Duties And Responsibilities/h3p1. Follow written and oral directions/pp2. Follow established performance standards and perform duties according to nursing service policies and procedures./pp3. Assist new employees in following established facility policies and procedures,/pp4. Identify resident problems and concerns and report them immediately to a charge nurse or a licensed nurse./pp5. Complete assignments timely, completely and accurately./pp6. Conduct resident rounds as assigned./pp7. Initiate corrective action as necessary and/or seek assistance of a licensed nurse./pp8. Identify safety hazards and emergency situations, and initiate corrective action./pp9. Attend all in-service classes as assigned and complete assignments./pp10. Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit./pp11. Document incidents and/or unusual problems according to established facility procedures./pp12. Participate in interdisciplinary team and other facility meetings as assigned./pp13. Participate in the development of an individualized plan of care of assigned resident./pp14. Review care plans and perform nursing care as outlined./pp15. Document in the nursing assistant notes the care and treatment provided to the resident and the residents response or lack of response to care provided./pp16. Report changes in residents condition to Charge Nurse or Nurse Supervisor./pp17. Follow facility procedures for admission, discharge and transfer of residents./pp18. Complete documentation accurately, timely and following facility policies and procedures./pp19. Listen to resident and family complaints and report problems to Charge Nurse./pp20. Assure residents have call lights at hand and answer call lights promptly./pp21. Assure residents are given adequate assistance with meals, nourishment and transportation to meal service area./pp22. Consistently work cooperatively with residents, charge nurses, treatment nurses, restorative nurse, other nursing assistants, physicians, families, consultant personnel and ancillary service providers./pp23. Observe all facility safety policies and procedures./pp24. Come to work in a clean, neat uniform and consistently present an appropriate professional appearance./pp25. Come to work as scheduled and consistently demonstrate dependability and punctuality./pp26. Assume accountability for data contained in the employees handbook./pp27. Perform other related duties as directed by his/her supervisor./pp28. Perform all duties assigned in an effective, timely and professional manner./pp29. Observe infection control procedures./pp30. Follow Residents Rights policies at all times./pp31. Be responsible for the safety of residents under his/her supervision./pp32. Observe all facility policies and procedures. 33.
