This position serves as a vital link between patients, their families, healthcare providers, and other team members.Key ResponsibilitiesPatient Care and Coordination: Assist in developing care plans and take appropriate actions to enhance patients' short- and long-term outcomes. Deliver care as outlined, using independent judgment when necessary. Collaborate and communicate care plans with relevant staff.Documentation: Maintain accurate records of patient assessments, changes in condition, care provided, education given, and any incidents related to care standards or hospital policies.Education: Act as a resource for patients and families on health-related topics, ensuring effective communication of necessary information to families and care providers.Patient-Centered Care: Embrace the principles of patient- and family-centered care to enhance satisfaction among patients, families, staff, and physicians. Prioritize actions based on patient needs and contribute to care plans by sharing relevant information while adhering to confidentiality standards.Performance Improvement: Offer suggestions for identifying solutions and support the implementation of changes aimed at improving patient quality outcomes, satisfaction, and organizational performance.
