The Coordinator of Day Habilitation Services is responsible to supervise the day-to-day operations of the Day Habilitation Program. Monday - Friday 8:30a - 4:30p Duties and Responsibilities E- Connotes Essential Functions E-1 Participates with the Assistant Director of Day Habilitation Services on the day to day administrative operations of the program which include: A. Supervises and trains the Day Habilitation Specialists. B. Determines staff schedules and caseload assignments for each Day Habilitation Specialist. C. Formulates activity schedules for the participants. D. Ensures that the daily attendance records are kept for all program participants. E. Ensures that the program’s facilities are maintained F. Approves and monitors all time sheets, time off requests and accruals of Day Hab Specialist. G. Provides necessary performance counseling to the Day Habilitation Specialists as necessary. H. Issues corrective actions to the Day Habilitation Specialists as the need arises. I. Completes performance reviews on the Day Habilitation Specialists annually. J. Completes all required in-service trainings. K. Maintains all financial ledgers, credit card transactions and petty cash receipts for the Day Program. E-2 Participates with the Assistant Director of Day Habilitation Services on the day to day programmatic operation of the program which includes: A. Ensures that the participants are provided with person centered planning to support and achieve outcomes such as increased sense of self-esteem, increase relationships with others, exercise of choice, development of new skills, improvement of existing skills, community participation and integration, personal enrichment, productive use of time and economic self-sufficiency. B. Ensures that the participants receive the necessary training in the area of maintenance and cleanliness of facility’s work areas and the safe use and care of the facility’s equipment. C. Ensures that necessary assistance is given to the participant, care manager, residential provider and advocate in order to formulate an individualized life plan or comprehensible functional assessment. D. Ensures the necessary documentation is maintained regarding the participant’s daily routines. E. Provides the necessary training in the area of activities of daily living, etc. E-3 Responsible to prepare monthly notes, staff action plans, life plans and completion of medisked billing requirements and deadlines. Ensure record books are up to date with OPWDD requirements. E-4 Enforces and adheres to all safety policies and procedures. 5- All other duties as assigned. Position Requirements and Qualifications Bachelor's Degree in related field preferred with one years’ experience working with the disabled population OR High School Diploma/GED with three years’ experience with IGHL Valid NYS driver’s license Complete and maintain AMAP, SCIP, CPR, IGHL Driver Authorization certifications as per IGHL policy Must be able to lift fifty pounds The Coordinator of Day Hab Services upholds IGHL’s mission, emulates the values and commits to the vision.
