This role functions as a clinical support and telehealth-based provider, partnering closely with on-the-ground clinicians to deliver high-quality, patient-centered care for individuals with chronic and life-limiting conditions. While this position contributes directly to clinical outcomes and program growth, it is not structured as a multi-state provider-of-record role, but rather as a high-impact remote clinical operator supporting care delivery, documentation, coordination, and billing readiness. Role Overview The Nurse Practitioner will: Support clinical decision-making and care planningDrive documentation quality for Medicare billingAssist in patient eligibility, enrollment, and care continuityCollaborate across care teams to improve patient outcomes and program efficiency This role is critical in enabling scalable, compliant, and high-quality care delivery across multiple markets. Key Responsibilities Clinical Support & Care Coordination Conduct telehealth assessments and follow-ups for palliative care patientsDevelop and maintain individualized care plans focused on quality of lifeSupport symptom management and medication oversight (within scope of licensure)Collaborate with local NPs, nurses, and physicians for in-person care executionParticipate in advance care planning discussions and documentation Program & Patient Growth Identify patients eligible for palliative care enrollmentSupport transitions from home health → palliative → hospice where appropriatePartner with internal teams to ensure continuity of care and patient retention Billing & Documentation Support Ensure all encounters are accurately documented for Medicare Part B billingMaintain timely, compliant EMR updates to support clean claimsAssist with CCM (Chronic Care Management) and RPM (Remote Patient Monitoring) documentationCollaborate with revenue cycle teams to reduce denials and delays Compliance & Quality Maintain documentation aligned with CMS, HIPAA, and state-specific guidelinesParticipate in quality assurance and audit processesSupport adherence to clinical protocols and regulatory standards Licensure & Certification Requirements Required Active Nurse Practitioner (NP) license in Arizona (AZ)National certification (FNP-C, AGPCNP-BC, or equivalent)Active DEA registration (or ability to obtain)Current BLS/CPR certificationExperience with Medicare Part B documentation and billing standards Preferred Experience in palliative care, hospice, or home healthFamiliarity with CCM and RPM programsPrior experience in telehealth or remote care models Education & Experience Master’s Degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)Experience managing medically complex patientsStrong understanding of clinical documentation, compliance, and care coordination Hard Skills EMR/EHR systems (Epic, Athena, eClinicalWorks, etc.)Medicare documentation standardsTelehealth platformsClinical documentation and care planning Soft Skills Strong clinical judgment in a remote environmentClear communication across distributed teamsHigh attention to detail and complianceAbility to operate in a fast-paced, multi-state program environment Why This Role Matters This role enables the organization to scale palliative care delivery efficiently while maintaining high clinical and compliance standards.
