Physical Therapy Assistant (PTA) – Home Health & In-Home Outpatient (IHOP)RVNAhealth | Southern Fairfield County, CTRVNAhealth is seeking seeking licensed Physical Therapy Assistants (PTAs) to join our team, with opportunities available in both Home Health and In-Home Outpatient (IHOP) settings.We are actively hiring for towns in Southern Fairfield County, including Stamford, Greenwich, Darien and Norwalk.We are hiring for full-time, part-time or per-diem opportunities to provide high-quality, one-on-one care to patients in their homes, supporting recovery, mobility, and independence.Position OptionsHome Health PTA (Medicare Part A)Work under the supervision of a licensed Physical Therapist to provide skilled, short-term therapy services to patients recovering from illness, injury, or hospitalization.Key Responsibilities:Deliver therapeutic exercises and treatments based on the PT's plan of careProvide hands-on care including gait training, strengthening, and mobility exercisesAssist patients with functional activities to improve independence and safetyMonitor and document patient progress; communicate updates to supervising PTParticipate in case conferences and interdisciplinary care planningMaintain accurate, timely documentation in compliance with Medicare Part A requirementsTravel locally to patient homes within assigned territoryIn-Home Outpatient PTA (IHOP – Medicare Part B)Provide ongoing, outpatient-level therapy services in patients' homes and assisted living communities, often supporting longer-term rehabilitation and post-surgical recovery.Key Responsibilities:Implement treatment plans developed by the Physical TherapistSupport patients recovering from orthopedic procedures and chronic conditionsProvide progressive strengthening, balance training, and mobility interventionsMonitor patient response to treatment and communicate with supervising PTAssist with adaptive equipment training and home exercise programsEnsure accurate documentation and compliance with Medicare Part B guidelinesDeliver consistent, patient-centered care in a home-based settingWhat You'll Do (Both Roles)Deliver compassionate, high-quality care focused on patient outcomesBuild strong relationships with patients and caregiversManage a productive caseload and travel within assigned territoryUtilize electronic documentation systems effectivelyCollaborate with therapists and interdisciplinary teamsQualificationsAssociate degree from an accredited Physical Therapist Assistant programCurrent Connecticut PTA license in good standing2+ years of clinical experience preferred (home health and/or outpatient a plus)Strong communication and teamwork skillsReliable vehicle and valid driver's license; ability to travel dailyComfort with technology and electronic documentationWhy Join RVNAhealth?Work one-on-one with patients in a meaningful, home-based settingSupportive, collaborative rehab and clinical teamCompetitive compensation and mileage reimbursementOngoing training and professional developmentBe part of a trusted non-profit serving the community for over 100 yearsBenefits of Working at RVNAhealthRVNAhealth is committed to providing an exceptional, supportive, and progressive work environment. Compensation: The pay range for this position is $38.00 - $42.00 per hour and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
