Are you living your life's work?**Position Description:**The Neuroscience Institute of Penn Medicine Lancaster General Health is seeking an APP to join its **Alzheimer's and Memory Care** practice. This is a unique specialty practice consisting of a Geriatrician, a Geriatric CRNP, a Physician Assistant, a Neurologist and Neuropsychology providers working collaboratively in an inter-professional environment to provide comprehensive care for patients experiencing memory loss.The Memory Center is experiencing increasing demand for services, in part due to several novel treatments becoming available to treat memory disorders. We will be adding a new practice location and expanding our team in 2026, and our looking for a motivated APP who is passionate about caring for people with memory loss to join the team.A Medical Director, Nurse Supervisor of Disease Management and Licensed Social Worker assist with program development as well as direct patient navigation to resources, education, and multiple support groups designed to address patient and caregiver needs throughout the stages of their cognitive disorder.The roles this new APP would serve in the program would include assisting with new patient evaluations, reviewing test results with patients and families, helping develop care plans, performing in-person and telemedicine follow-up visits to review care plans, and assist in managing behavioral and psychological symptoms related to memory disorders. The APP would also have opportunities to assist in support groups, assist in training of other health professionals, and get involved with quality improvement and program development at the Alzheimer's and Memory Care program.This team of clinical experts is excited to teach and mentor an interested and motivated APP with the desire to develop expertise in this growing field.**The position is an Outpatient only position.****Hours:** Full time.
