Job Title: Registered Nurse (RN) - Home HealthLocation: South Dennis, Massachusetts 02660Assignment Duration: 13 weeks (with possible extension)Schedule: 40 hours per weekShift: Day ShiftPosition SummaryThe Home Health Registered Nurse (RN) is responsible for delivering high-quality nursing care in a home care setting. This includes conducting comprehensive assessments of patients' physiological, psychosocial, and environmental conditions, along with reviewing health history to ensure safe and effective care. The RN develops, implements, and manages individualized care plans aligned with case management goals.Key ResponsibilitiesPerform comprehensive patient assessments at the time of admissionDevelop, implement, and regularly update individualized care plansIdentify patient needs and coordinate appropriate interventionsUtilize HHRG values to determine visit frequency and interdisciplinary service needsMaintain HHRG values and outcomes aligned with national benchmarksParticipate in interdisciplinary case conferences and evaluate care effectivenessProvide instruction and supervision to Home Health Aides every 14 days with proper documentationMonitor patient conditions and promptly report significant changes to physicians or care providersMaintain accurate, timely, and detailed clinical documentationObtain and clarify physician orders for all care servicesCoordinate patient discharges, including documentation and communication with care teamsRefer patients to appropriate community resourcesComplete all required documentation and administrative tasks on timePerform additional duties as assignedQualificationsActive Registered Nurse (RN) license in MassachusettsSuccessful completion of pre-employment medication testMinimum 1 year of nursing experience required (or 6 months with BSN)Current CPR certificationPreferred QualificationsBachelor of Science in Nursing (BSN)Prior home health or home care experienceIV therapy experienceStrong computer and clinical documentation skillsFor more details reach atAditi.sharma@navitashealth.comor Call / Text at 516-587-6677.
