Position Summary
The Medical Social Worker provides compassionate, patient-centered support to individuals and families receiving hospice services through Snowline Health. This role helps patients honor their goals and wishes, promotes dignity at end of life, and offers emotional, social, and resource-based support throughout the hospice journey. The Medical Social Worker collaborates closely with the interdisciplinary team to enhance comfort, quality of life, and overall patient and family well-being. Responsibilities include counseling, resource navigation, advocacy, and coordination with community agencies, along with maintaining timely documentation and meeting organizational productivity expectations. The ideal candidate demonstrates strong communication skills, empathy, professionalism, and the ability to adapt to the unique needs of patients, families, and the care environment.
Responsibilities include
Honor patient wishes and support them in achieving their goals
Help patients leave this world with dignity
Provide counseling and support, including coping strategies, anticipatory grief, and long-term care planning
Collaborate with the interdisciplinary team to ensure comfort and quality care
Connect patients and families to community resources and referrals
Communicate with social and community agencies, especially for high-risk families
Maintain documentation and productivity standards according to defined organizational expectations
Participate in team meetings, trainings, and community events
Demonstrate initiative, adaptability, cooperation, and dependability
Respect diverse beliefs, choices, and lifestyles
Perform additional duties as assigned
Requirements
Master of Social Work (MSW) degree from a graduate school accredited by the Council on Social Work Education
Licensed Clinical Social Worker (LCSW) preferred
Minimum of one (1) year of clinical geriatric experience required; hospice experience preferred
Demonstrated ability in casework and counseling
Strong communication and counseling skills
Demonstrated skills in assisting individuals and families with problem-solving and navigating community resources
Ability to efficiently operate a computer; prior Electronic Health Records (EHR) experience preferred
Valid California driver's license and a reliable, insured vehicle
Comfortable being mobile and traveling throughout the service area
Open to an occasional flexible schedule, including evenings or weekends
Salary Description
$37 - $54 hr DOE
