Wound Care Nurse Practitioner
Overview
We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team. The ideal candidate will possess extensive knowledge and experience in wound care management, demonstrating a commitment to providing high-quality patient care. This role involves assessing, diagnosing, and treating patients with various wound types while collaborating with interdisciplinary teams to ensure optimal patient outcomes.
Responsibilities
Conduct thorough assessments of patients with acute and chronic wounds.
Develop and implement individualized care plans based on patient needs and evidence-based practices.
Perform wound debridement, suturing, and other necessary procedures to promote healing.
Educate patients and families on wound care management and prevention strategies.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive care.
Maintain accurate medical documentation in compliance with HIPAA regulations.
Participate in clinical research initiatives related to wound care practices.
Stay current with advancements in wound care treatments, including botulinum toxin treatment and esthetic laser treatment.
Perform skin substitute applications as medically necessary
Adhere to CMS rules and Regulations in regards to conservative wound treatment guidelines and protocols.
Skills
The successful candidate will possess a diverse skill set that includes but is not limited to:
Proficiency in EMR/EHR systems such as Epic, Athenahealth, Cerner, eClinicalWorks.
Experience in critical care, hospital medicine, emergency medicine, internal medicine, or outpatient settings.
Knowledge of ICD-9/ICD-10 coding and CPT coding for accurate medical documentation.
Familiarity with acute pain management techniques and behavioral health considerations.
Ability to perform procedures such as IV infusion, catheterization, venipuncture, and specimen collection/processing.
Understanding of geriatrics, pediatrics, hospice & palliative medicine, and memory care practices.
Strong patient assessment skills with an emphasis on anatomy knowledge and physiology knowledge.
Experience working in various healthcare environments including nursing homes, assisted living facilities, urgent care centers, level I/II trauma centers, teaching hospitals, and home care settings.
