JOB SUMMARY
The Health Case Manager (HCM) exists to ensure all people supported have the health services and supports needed to thrive. HCM’s monitor services and supports for quality and continuity. They are responsible for maintaining all documentation related to health services, supports, and oversights to ensure all needs for persons supported are met. All duties are performed with the knowledge that Breakthrough strives to be the model of excellence for adults with autism. This position will report to the Health and Individual Rights Coordinator.
PERFORMANCE REQUIREMENTS
A. General Responsibilities
1. Collaborates with the Residential Services Managers in scheduling medical appointments for people supported and provide assistance when needed to ensure the person attends the appointment.
2. Communicates regularly and effectively with management and direct support staff to ensure quality of care.
3. Attend medical appointments with person supported when needed.
4. Ensure all documentation from medical appointments is complete and uploaded into the electronic health record (Therap).
5. Ensure all follow ups for medical appointments are scheduled and documented appropriately in the electronic health record (Therap).
6. Monitor Medication Administration Records (MAR) for compliance, documentation, and accuracy.
7. Monitor medication closets for cleanliness and compliance monthly.
8. Check in medications as needed
9. Administer medications as needed
10. Enter new or changed medications on the MAR.
11. Ensure medication variances are completed accurately and in a timely manner.
12. Create and monitor menus for homes in collaboration with the designated residential services manager.
13. Ensure persons supported are encouraged to follow their nutrition guidelines.
14. Attend COS meetings for people with high health needs.
15. Train Direct Support Professionals (DSP) on health needs of the people supported as needed.
16. Encourage and promote enabling technology into the lives of the people supported.
17. Perform direct care responsibilities as needed.
18. Abides by all policies, procedures, regulations and guidelines for safety, quality assurance and general corporate oversight of Breakthrough and/or DDA.
19. Perform other duties and responsibilities to meet the needs of Breakthrough and/or the persons served as needed.
20. Maintain open lines of communication with family members, conservators, ISC’s, DDA staff, and others related to the continuity of supports and services.
21. Ensure all services and supports are delivered in accordance with the ISP and Cost Plan summary according to Breakthrough and/or State of Tennessee, Department of Disability and Aging (DDA) procedures and policies.
22. Maintain contact with all therapy providers and provide documentation of contacts.
23. Attend all meetings as necessary.
Supervisory Responsibilities
24. Assist in training and mentoring DSPs.
25. Assist in providing progressive discipline along with counseling and constructive feedback to DSPs.
26. Assist with performance evaluations of DSPs.
27. Maintain a safe work environment by identifying and correcting potentially unsafe conditions and work practices.
28. Maintain a positive working climate within and across departmental functions.
B. Performance Responsibilities
29. Must be able to attain and remain current with Medication Administration for Unlicensed Personnel training certification provided by the agency.
30. Support the organization’s mission, vision, and philosophy.

B. Physical Demands
Must have good physical and mental health and be able to work in a fast-paced work environment. Must be able to deal with stress and adjust to a flexible work schedule. Should be able to work for periods of over 2 hours while standing and safely lift at least 25 pounds over their head. Must be physically able to be trained in Professional Crisis Management.
C. Special Demands
The Health Case Manager must: have understanding, patience and tact in dealing with persons served, their families or advocates and other agencies involved in providing supports for persons served; the ability to maintain good working relationships with staff and with contacts from other agencies or entities; be able to prioritize work tasks under the general direction of the Executive Director; and be able to work without close personal supervision.
D. Qualifications
Bachelor’s degree preferred in healthcare, psychology, social work, special education, social work, occupational therapy, behavior analysis, or a closely related field; will accept an Associate’s degree; extensive work experience may be substituted. Will accept at least one year paid work experience in a healthcare, case management or similar social services position, previous supervisory experience preferred, and at least one year experience working with individuals with autism and/or developmental disabilities.
E. Job Knowledge
The Case Manager must possess strong interpersonal and communication skills and ability to work effectively with a wide range of individuals. They must have the ability to communicate both orally and in writing and maintain composure in difficult situations. The Case Manager must be able to read and understand instructions, perform adequate record keeping and write reports as necessary. They must have the ability to coordinate and delegate responsibilities and have the ability to maintain objectivity when dealing with staffing issues and uphold all company policies. The Case Manager must also successfully complete and actively maintain all required trainings including but not limited to First Aid, CPR, Relias, Medication Administration for the Lay Professional, and Professional Crisis Management.
The Case Manager should possess a working knowledge of Medicaid guidelines and State of Tennessee, Division for Department of Intellectual and Developmental operating principles. They should have a working knowledge of the support needs of individuals with autism spectrum disorders as well as the skills and training necessary to implement, oversee and troubleshoot structured interventions to meet these needs. The Case Manager will be expected to seek out and attend continuing education opportunities as approved and required by the Executive Director.
G. Employment Variables
Evening and weekend work may be required to meet the needs of the individual persons served; Available to perform direct care responsibilities when needed; Ability to work one weekend day per month; will be in constant contact with persons served, their families and other support agencies. Some travel required. Cannot have secondary employment unless approved by the Executive Director and permission for secondary employment may be rescinded at any time.
H. Must have a good driving record and reliable transportation.
