One of our main goals at CNY Family Care, LLP is to provide all of our patients with continuous and comprehensive health care. As part of this care model, we ask all of our patients to select one personal physician. This PCP/Primary Care Physician will maintain an ongoing relationship with the patient and will then always be in charge of their care. They will oversee the care team that consists of nurses, Nurse Practitioners, Physician Assistants and Diabetic Educators who might interact with the patients when they visit the office. Our care team will work towards ensuring that the patients receive the appropriate age-related preventative testing and treatment as well as the latest evidence-based treatment (proven and documented) for their individual medical condition(s).We continue to grow and expand our services in order to meet the demands of our patient population.
