Community-Based Services Social Worker

Crouse Community Center is seeking a compassionate, motivated Social Worker to support our Adult Day Health Centers in Morrisville and Sherburne and our Supportive Housing program at Community View Apartments in Morrisville. This is a meaningful role for someone who wants to make a real impact in the lives of older adults and individuals with disabilities.   Main Function: To perform duties related to admissions, transfers and discharges of registrants from Senior Services. Assist registrants and tenants to maintain a productive, purpose-filled life by professional counsel and in the resolution of, and/or adjustment to their economic, emotional, social and physical needs, maintain effective relationship with family members to meet these needs; maintain effective interaction with the staff of Senior Services, registrants/tenants, community agencies and resources.    This position is required to work a flexible work location schedule that accommodates the needs of the registrants and tenants. 80% of work week will be at the Adult Day Health Center (Morrisville and Sherburne) and 20% at the Supportive Housing at Community View Apartments (Morrisville).   Major Responsibilities (To serve as general overview, subject to evolve to meet organization needs)1. Provide intake interviews and applications, referrals as indicated, pre-admission screening, and assistance with discharges within programs2. Assist in promoting community-based services with potential clients and the public. 3. Assist in resolution of initial adjustment, interpersonal relationships, policies and procedures, financial and special needs problems. This assistance must be coordinated with the client’s family, their adult home and appropriate community organization.4. Act as a liaison between individuals and community agencies with client’s family or their adult home. Maintain contact with registrant relatives, adult home or Public Health regarding problems and status.5. Maintain appropriate case records; social histories, progress notes, admission and discharge summaries, care planning and compile statistical reports as required.6. Maintain awareness of new concepts and developments in social work techniques, particularly as applied to gerontology and implementation. Good knowledge of DOH Policies including Medicaid.7. Participate in appropriate staff and care plan meetings and consultations.8. Actively participate in the development of the monthly community integration calendar.  Education: BSW, Human Services Degree or related field. Should have the maturity, patience, and empathy to deal with the elderly and disabled, and their families under varying circumstances and stress.
