Quick FactsPay: Negotiable based on experience, knowledge, skills, and abilitiesLocation: Orlando (Orange Co.)Schedule: PRN, successful candidates will have 2-3 days of availability weekly from 7:00a-4:00pEmployment type: W-2 or 1099Mileage: Currently $0.60 per mile (W-2)Reports to: Branch Director What You'll DoAssess each patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential, including the home environmentCarry out the physical therapy plan of care set by the physicianInstruct patients and families on therapeutic appliances and prosthetic devicesDocument care accurately and on time, including clinical and progress summariesCoordinate with physicians and report patient response to treatment or changes in conditionIdentify needs for other home health services and refer as appropriateParticipate in discharge planning and case conferencesProvide consultation to families/caregivers and inservice education to organization personnelSupervise physical therapy assistants per organization policy and state regulationsCheck in on home health aide care every two weeks when therapy is the only skilled serviceTake on other duties as assigned QualificationsPossesses a degree from a baccalaureate or Master's program in physical therapy approved by an accredited organization.Licensed to practice as physical therapist within the state, if applicable.Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.Demonstrates good verbal and written communication and organization skills.Possesses and maintains current CPR certification.Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
