A company is looking for a Community Health Worker (Remote - Upstate Region). Key Responsibilities Provide guidance and support for members through home visits and care coordination Assist with member engagement, onboarding, and facilitate referrals to clinical care programs Support community outreach and promote programs through participation in community events Required Qualifications, Training, and Education High School Diploma or equivalent 3 years of work experience, including 1 year of community involvement or customer service experience Understanding of basic principles of Medicaid programs and knowledge of community resources Community Health Worker certification within 6 months of hire Proficiency in Microsoft Office
