Social Worker
White House Healthcare Center, a privately owned skilled nursing facility in Orange NJ is seeking a Part Time Social Worker to join their team of highly skilled professionals. Social Worker responsibilities include but are not limited to:
Develop preliminary/comprehensive assessments for each resident.
Report any issues/ changes to the Director of Social Services.
Encourage the resident/family to participate in the developmental process and review of resident’s plan of care.
Develop a written plan of care for every resident that identifies all social problems and needs of the resident, followed by setting goals to be accomplished for each problem/need identified.
Ensure that all social services components of the MDS reports are completed and signed in the most efficient manner.
Review nurse’s documentation to determine if the care plan in place is being followed properly.
Job Type:
24 hours a week between the hours of 9-5
License/Certification:
1 year of nursing home social worker experience is preferred
CSW or LSW required
