Job Description Job Title: Home Health LPN (Contract-to-Hire) Pay rate : $37.30 an hr Location: On-site / field-based — Clarksburg/Owings, WV area (coverage: Harrison, Marion, Tyler, Barbour, Doddridge, Lewis, Upshur counties) Schedule: Mon-Fri 8:00 AM-5:00 PM + evening/weekend on-call rotation Type: W2 Contract-to-Hire | 40 hrs/week Mileage Reimbursement: Yes Productivity Expectation: ~ 32 points/visits per week Job Summary We're hiring a Home Health LPN to provide in-home skilled nursing care in accordance with the patient's plan of care, while supporting patient dignity, independence, and safety. This is a contract-to-hire opportunity with a strong, mission-driven healthcare team. Responsibilities * Provide skilled nursing care per individualized plan of care and physician orders * Monitor vital signs and report abnormal findings * Administer medications, feedings, oxygen, ostomy care, and other treatments as ordered * Coordinate routines established by PT/OT/Speech and other care professionals * Maintain infection control standards; identify and report communicable disease/infection concerns * Document all care provided and patient progress toward goals accurately and timely * Order/maintain adequate supplies, medications, and feedings * Monitor incidents, assess patients post-incident, and ensure incident reporting/notification is completed * Ensure medical equipment is maintained; report faulty equipment for repair Required Qualifications * Active West Virginia LPN license * 1+ year Home Health experience (required) * Homecare Homebase experience (required) Preferred / Nice to Have * OASIS experience (not required) What You'll Get * Contract-to-hire pathway * Mileage reimbursement * Consistent weekday schedule with rotating on-call support How to Apply Apply with your resume and include: * Confirmation of WV LPN license * Home Health experience details * Homecare Homebase experience Equal Opportunity Employer.
