Oregon Licensed Clinical Pharmacy Specialist
To elevate patient care at a nationally recognized Academic Medical Center, the remote Oregon Licensed Clinical Pharmacy Specialist will provide comprehensive medication management, collaborate with care teams under a Collaborative Practice Agreement, and optimize therapy for patients.
Key responsibilities
Provide comprehensive medication management, including reconciliation, dose optimization, and adherence support
Partner with physicians and interdisciplinary teams to order individualized care plans and ensure seamless medication coordination
Monitor therapy effectiveness and safety, identifying optimization opportunities and intervening as necessary
Required qualifications
Doctor of Pharmacy (PharmD) degree
Licensed pharmacist in Oregon
Completion of a PGY2 Ambulatory Care Residency or PGY1 Residency with at least 1 year of ambulatory practice under a CPA, or at least 3 years of ambulatory practice with disease state management
Strong communication, organization, and technology skills
