Registered Nurse (RN) Home Health Case ManagerTo The Rescue is seeking a Registered Nurse (RN) Home Health Case Manager to provide skilled nursing care and care coordination for adult patients in their homes. This role offers meaningful one-on-one patient care, professional autonomy, and a healthier work-life balance compared to hospital or facility-based nursing.You'll manage patient care from admission through discharge, working collaboratively with physicians, therapists, patients, and families to help individuals remain safe, independent, and comfortable at home.
