Job Summary
The Clinic Advanced Practitioner Provider (APP) is a certified, licensed nurse practitioner or physician assistant who independently and in collaboration with supervising physicians provides specialized care to chronically ill patients in the Clinic setting.
Essential Job Functions
Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of medical conditions
Demonstrates and uses critical thinking skills to synthesize and analyze patient data.
Performs a problem-focused history and physical assessment for adult patients that is accurate and complete.
Orders, collects, and correctly interprets and evaluates laboratory and/or other diagnostic tests to assess patient problems and health care needs.
Prescribes and manages pharmacologic agents and/or other treatment modalities with attention to efficacy and safety.
Identify patients age specific needs and alter care plans appropriately
Formulates and implements comprehensive outcomes-based treatment plans using the patient assessment, evidence-based medicine, standards of care, professional practice guidelines, and in collaboration with the interdisciplinary team, when appropriate.
Evaluates patient responsiveness to treatment therapies and adjusts the treatment plan, as indicated.
Facilitates and maintains ongoing communication with the patient, family, and/or health care team members.
Counsels and educates patients and families regarding medical processes and treatment.
Maintains accurate, complete, and timely documentation in the electronic medical record.
Conduct yourself in a prudent manner in line with your peers. Deviation will be subject to internal (DPACC) and external (facility) based Peer Review Process for any medical decision making, competence, mental judgment, or behavior related incidents or concerns that have been called to question
Requirements
Graduate of accredited Nurse Practitioner or Physician's assistant program
Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
Preferred experience of working at least one (1) year in the clinic setting as a NP or PA
Strong working knowledge of given clinic specialty
Excellent verbal and written communication skills.
Ability to prioritize tasks.
Ability to work as a member of a multidisciplinary team.
Must be able to work independently with appropriate physician collaboration
