This leadership role is an excellent opportunity for a motivated professional to impact patient care, enhance program effectiveness, and foster a supportive, compliant, and patient-centered care environment.Position SummaryThe Social Work Manager will oversee social work operations including assessments, care planning, compliance, and delivery of interventions. The role includes managing staff, collaborating with interdisciplinary teams, and developing programming that supports patients and their families. This position emphasizes clinical expertise, leadership, and organizational effectiveness, with a focus on improving the well-being of elderly and frail populations.Core ResponsibilitiesManage and supervise Social Work Supervisors, Social Workers, Behavioral Health Specialists, and Social Work Assistants.Hire, train, supervise, and coach social work staff to maintain high standards of care.Oversee quality of assessments, interdisciplinary team (IDT) participation, and patient care plan execution.Ensure compliance with documentation, assessments, care plans, participant rights, and grievance processes.Collaborate with Behavioral Health Specialists to develop support groups, counseling services, and family education programs.Partner with community professionals to extend care resources and services.Participate in end-of-life care coordination and support.Conduct home visits as needed to provide direct oversight and patient engagement. Skills & QualificationsMaster's degree in Social Work (MSW) required.Social work licensure preferred.Minimum 2 years of supervisory or leadership experience in social work strongly preferred.Minimum 1 year of experience working with frail or elderly populations.Strong organizational, clinical, and communication skills.Valid state driver's license with a clean driving record.Proof of current auto insurance required.
