What Will You Do:Conduct initial and periodic physical therapy evaluations to determine residents' functional status and establish appropriate treatment plans.Implement individualized treatment plans based on physician orders, assessment findings, and resident-centered goals. Deliver skilled physical therapy interventions to improve residents' mobility, strength, balance, endurance, and functional independence.Modify and progress treatment activities based on resident response, with a focus on maximizing functional outcomes and promoting safety and quality of life.Accurately document treatment sessions, resident response, and clinical observations through daily notes, progress reports, and discharge summaries in accordance with facility policy and regulatory standards.What We Are Seeking:Graduate of a Physical Therapy curriculum approved by the American Medical Association.
