Franklin Restorative Care Center - Physical Therapist (PT)Position Summary:The Physical Therapist (PT) is responsible for evaluating, planning, and implementing physical therapy services to restore function, improve mobility, relieve pain, and prevent or limit permanent disability. The PT collaborates with the interdisciplinary team to ensure individualized, resident-centered care that promotes health and independence.Key Responsibilities:Evaluate patients' physical abilities, needs, and goals through assessments and screeningsDevelop individualized treatment plans in accordance with physician orders and clinical best practicesProvide direct physical therapy treatments including therapeutic exercise, gait training, balance, and mobility interventionsTrain patients, families, and caregivers on safe mobility, home exercise programs, and use of assistive devicesMonitor and document patient progress, updating treatment plans as needed to maximize outcomesCollaborate with physicians, nursing, and other therapy professionals to ensure coordinated careMaintain compliance with federal, state, and professional practice standardsSupervise and guide Physical Therapy Assistants (PTAs) as appropriateQualifications:Current and active Physical Therapist (PT) license in MinnesotaGraduate of an accredited physical therapy programStrong clinical skills with knowledge of rehabilitation techniques, equipment, and safety standardsExcellent communication, documentation, and interpersonal skillsAbility to work effectively in a team environmentPrior experience in skilled nursing, rehabilitation, or long-term care setting preferredWhat's Offered:Competitive pay and benefits packageOpportunities for continuing education and career advancementSupportive, team-oriented environmentA rewarding opportunity to make a lasting impact on patients' mobility, independence, and quality of life
