Nurse Practitioner

CHM Mobile Care, is a Shelbyville, KY based Medical Provider group focused on direct patient care specifically transiting from acute to post-acute. We are seeking a qualified Nurse Practitioner in the Lexington, KY area to provide care Monday - Friday in person and via telehealth. At a minimum, one day per week will be seeing patients in a long-term care facility to address wound and primary care.In this role, you will evaluate each patient based upon the received discharge orders and assess for current conditions, chronic conditions, and either continue or formulate an appropriate treatment plan. Each patient is to be seen within 72 hours of discharge and followed up with frequently during the first week. Additional visits are based upon response to treatment and medical needs.To qualify, you will have:MSN or DNP from an accredited university.Active and unencumbered Nurse Practitioner license to practice in Kentucky or obtain.Active and unencumbered DEA license to prescribe in Kentucky or obtain.Active BLS or ACLS certification.Experience providing direct patient care as a Nurse Practitioner.Familiarity with EMR systems, knowledge of Tebra (formerly Kareo) is ideal.Excellent communication and collaboration skills.the ability to provide availability each week of at least two days.Compensation is based on $85 per patient visit plus monthly travel stipend and medical professional liability coverage.
