This role ensures compliance with facility policies while delivering compassionate, resident-centered care.Essential Duties & ResponsibilitiesProvide nursing services in accordance with scope of practice, facility policies, and professional standardsMonitor residents for acute changes in condition and notify providers as appropriateMonitor and manage chronic health conditions and report significant changesAssist with admissions, resident orientation, and discharge planningEnsure physician orders are in place for newly admitted residentsParticipate in MDS assessments (admission, quarterly, change of condition, etc.)Contribute to individualized, resident-centered care plans with the IDTProvide culturally competent, trauma-informed nursing careMonitor resident weight, nutrition, and hydration statusSupervise and provide guidance to CNAs as directed by the Director of NursingMaintain accurate nursing documentation in EMR systemsCollaborate with hospice and palliative care teamsPromote fall prevention and resident safety initiativesAdminister medications and monitor for adverse effects or errorsTranscribe provider orders accuratelyAssess and manage resident painAssist with ADLs and resident repositioning as neededMonitor skin integrity and provide wound care as orderedCollect specimens for laboratory testingAdhere to infection prevention and control protocolsQualificationsGraduate of an approved RN or LPN programCurrent nursing license in good standing (Illinois)CPR certification requiredLong-term care experience preferredPhysical & Sensory RequirementsAbility to move throughout the workdayAbility to read, write, speak, and understand EnglishAbility to manage mental and emotional stressAbility to function independently with flexibility and professionalismAbility to lift, push, pull, or move at least 25 poundsAbility to assist with resident evacuation in emergenciesReasonable accommodations may be made to enable individuals with disabilities to perform essential functionsIND123
