DescriptionPosition DescriptionRegistered DietitianPrimary Function:The Registered Dietitian shall provide needed services under the direction of the primary physician and/or medical director in accordance with the plan of treatment and patient/family care plan.Supervision: Chief Clinical OfficerQualifications:An active, appropriate license and certification to practice as a Registered Dietitian in North Carolina.Ability to work with an interdisciplinary team.Ability to establish realistic goals and objectives appropriate to the palliative rehabilitation of terminally ill patients and to be sensitive to the needs of the dying patient.Duties:The following are the specific responsibilities of the Registered Dietitian:1. Provides consultation and information on diet to the Director of Nursing or designee for patients or families having nutritional problems and who are eligible for services of Hospice of Davidson County, Inc.2. Maintains records of consultations, visits, and progress in therapy achieved on forms provided by Hospice of Davidson County, Inc. for each patient consultation on a per-visit basis.3. Observes, records, and reports to the physician, nursing staff, and interdisciplinary team the patient's reaction to treatment and any changes in the patient's condition and expected duration of instruction.4. Provides materials when working directly with patient/families receiving instruction. Materials to be used outside nutritional visits are to be provided by the patient/family.5. Attends pertinent patient/family team conferences for the purpose of reviewing and planning patient care, scheduling visits, policy formation, and for quarterly auditing of patient records as required by the Division of Facility Services.6. Adheres to the policies and procedures of Hospice of Davidson County, Inc.7. Maintains confidentiality of Protected Health Information (PHI) at all times.Physical Demands:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
