We are looking for a skilled Nurse Practitioner to provide comprehensive primary care services to residents of assisted living facilities. The ideal candidate will have experience with the geriatric population and/or long-term care, and a commitment to delivering high-quality, patient-centered care.Essential Duties and ResponsibilitiesConduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.RequirementsMaster s degree in Nursing and graduate of an accredited Nurse Practitioner program.Unrestricted New York State Nurse Practitioner license.Unrestricted Drug Enforcement Administration (DEA) permit to prescribe all medications commonly prescribed in the course of the practice.Eligible to provide reimbursable services and maintain provider status with Medicare andMedicaid.Minimum of two years clinical experience as an NPPreferred Experience:Experience with eClinicalWorks (eCW) electronic health records.
