Registered Nurse (RN) – Behavioral Health

Make a meaningful difference in your community.Easterseals PORT Health is seeking a full-time Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. In this role, you’ll provide essential, community-based care to adults living with serious mental health and substance use challenges.This is not traditional facility-based nursing. It’s impactful, hands-on behavioral health work serving individuals where support matters most – in homes, shelters, and community settings.Voted Raleigh’s Best Nonprofit and Best Mental Health Services two years in a row!Your Role in Our MissionAs an ACT Team RN, you will work within a multidisciplinary team delivering community-based behavioral health support. You’ll provide psychotropic medication management, holistic nursing assessments, care coordination, and crisis response — all aimed at helping individuals achieve stability, independence, and improved quality of life.You will develop meaningful relationships with clients, support them through real-life challenges, and ensure seamless continuity of care across community environments. Community travel is required.Compensation & BenefitsWe offer comprehensive benefits for benefits-eligible positions.Salary Range: $75,000 – $90,000 annually, based on experienceBenefits IncludeMedical, Dental, & Vision InsurancePaid Time Off + 11 paid holidays403(b) Retirement PlanCompany-paid Life & Disability InsuranceMileage reimbursementCompany-issued phone, laptop, and full IT supportEmployee Assistance ProgramEligibility for Public Service Loan Forgiveness (PSLF)What You’ll DoMedication & Clinical CareAdminister and monitor psychotropic medications as prescribedConduct nursing assessments and track symptoms, side effects, and medical issuesDocument all care and observations in the Welligent EMRCare CoordinationPartner with the team psychiatrist to update medication protocols and care plansParticipate in daily in-person ACT Team meetings to ensure coordinated, person-centered treatmentCrisis ResponseProvide crisis assessment and intervention as part of a rotating on-call scheduleCommunity-Based SupportDeliver nursing care in clients’ homes and community settingsEducate individuals on health management, wellness, and medication adherenceAdvocate for client choice and connect individuals with needed community resourcesWhat We’re Looking ForTo be successful on the ACT Team, you should be compassionate, flexible, and comfortable providing care in diverse community environments.Required QualificationsActive, unrestricted RN license in North CarolinaExperience in behavioral health or mental health (preferred)Comfort working in community-based settingsValid driver’s license, reliable transportation, and vehicle insuranceStrong communication, documentation, and relationship-building abilitiesReady to Apply?Join a team where your clinical skills and compassion truly matter.
