Contract Speech Therapist Home Health

Healthcare Executive Placements, Inc. (HCEPInc) is seeking a Contract Speech Therapist (SLP), Home Health for a W2 Contract position based in Santa Clara, CA.Schedule: Full time, Monday through Friday, 8:30am - 5:00pmAssignment Duration: 13 Weeks (possibility of extending)Compensation:This position offers an hourly rate of $30 to $35 per hour, which equates to $1,200 to $1,400 per week (based on a 5-day / 40-hour workweek).Per Diems: Lodging: $192 per day and Meals & Incidentals: $92 per dayPer diems are paid daily, 7 days a week, totaling $1,988 per weekTotal Weekly Compensation (Hourly + Per Diems): Between $3,188 to $3,388 per weekBenefits: Paid sick leave, Paid holidays, Co-paid medical, dental, and vision insurance, Cafeteria plan access and 401(k) retirement plan.Summary:Provides skilled speech-language pathology services in the home health setting to patients experiencing communication, cognitive, and swallowing disorders. Ensures delivery of high-quality, patient-centered care that complies with local, state, and federal regulations, as well as agency standards of practice.Essential Functions:Provides comprehensive speech, language, cognitive, and swallowing evaluations in the home health environment.Develops and implements individualized treatment plans with measurable goals based on physician orders.Delivers therapeutic interventions to address speech, language, voice, fluency, cognition, and dysphagia.Monitors patient progress and modifies treatment plans as clinically appropriate.Educates patients and caregivers on therapy techniques and home exercise programs.Obtains and documents vital signs as required.Completes accurate and timely documentation in EPIC.Collaborates with physicians, nurses, and interdisciplinary team members to coordinate patient care.Maintains compliance with infection control, safety, and regulatory standards, including fit testing requirements.Qualifications:Current Speech-Language Pathologist (SLP) license required.Current AHA BLS certification required.Fit testing clearance required.Active driver's license required.Experience in home health preferred.Proficiency with EPIC charting system required.Ability to independently manage caseload and travel within assigned territory required.About Healthcare Executive Placements Inc.
