Join a traveling Physical Therapist role in Ontario, OH providing home health care where you will help patients recover from injuries and manage chronic conditions while experiencing a new location. Apply now and a recruiter will reach out with more details. Minimum Requirements Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Current Physical Therapy license in Ohio required to start the assignment. Current CPR certification required. Must have a valid driver’s license, vehicle insurance, and access to a dependable vehicle for travel throughout the service area. Experience providing home health physical therapy and completing CMS-required documentation and evaluations. Ability to develop and implement individualized PT plans of care, perform progress notes per agency policy, and prepare written discharge or summary reports at least every 60 days. Willingness to participate in weekend rotation and potential availability on observed holidays as needed. Must be able to visit patients across the agency’s service area and nearby affiliate service areas as needed (floating between sites may be required). Preferred: Experience supervising PTAs and home health aides in accordance with state regulations and agency policy. Assignment Details Primary Specialty: Physical Therapist.
