Lead Case Manager-DAAA
Plan, develop and coordinate activities to help the Community Council/Dallas Area Agency on Aging work toward ensuring integrated and sustainable programs. These programs include, but not limited to case management, CDSME education, care coordination, care transitions, worksite wellness programs, caregiver support and other programs as they are added under CCM Medicare Part B, health plans and private pay programs.Essential Functions-Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. General Responsibilities:Manage and secure new contracts as it relates to CCM within the Evidence-Based programs.Provide individual assessment for all clients referred under the CCM program.Develop a care plan as needed to coordinate services and appropriately documented CCM will ensure:Compliance with agency confidentiality, privacy and security proceduresProper data collection and tracking of performance outcomes CCM will ensure programmatic oversight of all programs as follows:adequate recruitment of targeted population participants, volunteers and targeted sitesCompliance with all programs and fidelity requirementsParticipate in the NCOA Learning Collaborative to learn more about the CCM and the (NCQA)Assist with and/or manage program budget, expenditures and assist with developing and forecasting program budget for CCM programsInterpret Medicare policies and procedures, laws, etc., makes independent judgements and determinations when needed regarding Medicare submissions, reimbursements and referrals Train in Evidence-based programs and teach classes as needed and/or appropriate.Ensures that all program activities are operating in accordance with Texas Health and Human Services Commission regulations and the Older Americans Act.Prepare and revise, as appropriate, separate schedules of activities including meetings, classes, trainings, marketing, coordination with stakeholders, milestones and plan for evaluation. Assist as needed with identification and research of grant opportunities and proposal writing, where needed.Maintain compliance with agency confidentiality, privacy and security procedures. Represent the agency as an official spokesperson and actively participate in community activities as assigned.Support the DAAA Senior Director in the development of new business opportunities around CCM as appropriate.Develop worksite wellness programs and site locations to provide funding opportunities.Perform other related duties as may be assigned.Skill Competencies-Excellent communication skills—ability to speak and write effectively, public speaking experience a plus; fluency in a second language (Spanish, Mandarin Chinese, Vietnamese, Korean, Farsi) a plus but not required.Basic knowledge of computers; advanced Word, Excel, Powerpoint and Outlook skills.Highly organized, ability to multi-task and prioritize activities.Knowledge of benefits, services and programs serving older adults and/or adults with disabilities and experience working with those populations.Case management experience, as appropriateDemonstrated skill in providing direction and leadership to program staff, when requiredAbility to work under pressure, meet deadlines, problem solve in an environment of multiple projects, rapidly shifting deadlines and disruptive urgencies. Ability to work independently without close oversight with a strong work ethic. Ability to productively engage with others at varying levels of seniority within and outside CCGDSupervisory Responsibility-None at this time, but may be added as program demands Work Environment-This job operates in a professional office setting. This role routinely uses standard office equipment such as computers, printers, scanners, phones, photocopiers, fax machines.Physical Demands-This is a combination of office and in the field largely and mobility is required. Ability to lift,bend or stand as necessary and ability to transport materials to and from meetings, presentations and to home visits as needed. Good vision, and good hearing acuity (with corrective lenses and/or hearing aids if necessary).Position Type/Expected Hours of Work-This is a full-time position. The days and hours of work are Monday through Friday, 8:00 a.m. to 5:00 p.m. Occasional evenings and weekend work may be required as job duties demand.Travel-This position requires less than 40% travel. Must have reliable transportation with a current valid Texas driver's license and state mandated automobile insurance for travel to and from meetings and presentations.BS (Bachelor of Science) and/or in social services related field or five years’ experience in the field.
