The Physical Therapist will evaluate and treat patients to improve mobility, reduce pain, and restore functional independence while collaborating with the interdisciplinary care team.ResponsibilitiesEvaluate patients and develop individualized physical therapy treatment plans.Provide therapeutic exercises, mobility training, balance training, and functional rehabilitation.Implement treatment plans and select appropriate therapeutic modalities to meet established goals.Monitor patient progress and modify treatment plans as needed.Provide patient and caregiver education to support continued recovery and safety.Assist with discharge planning and recommendations for continued therapy or equipment.Maintain accurate and timely patient documentation in accordance with facility guidelines.Collaborate with physicians, nurses, and therapy team members to coordinate patient care.Maintain department productivity standards.Ensure therapy services meet age-specific, cultural, and rehabilitative needs.Required SkillsStrong written and verbal communication skillsAbility to work effectively with interdisciplinary healthcare teamsAbility to prioritize tasks and manage time efficientlyKnowledge of physical therapy rehabilitation techniques and modalitiesEducationRequired: Doctor of Physical Therapy (DPT) or graduate of an accredited Physical Therapy programLicensure / CertificationRequired: Active Physical Therapist license in the state of assignment or ability to obtainRequired: American Heart Association (AHA) BLS CertificationExperiencePreferred: Skilled Nursing Facility (SNF) or rehabilitation experienceAbout Towne U.S. LLCTowne U.S. is a proud member of the Towne Family of Companies, which has been building community and opportunity since 1991.
