Board Certification is required
Active Indiana license is required
Dates needed 12/25-12/30 (7AM-7AM); temp privileges approved
Ongoing vacation coverage in 2025 for solo surgeon
Hospital coverage; 5 patients per day; NP support staff
CABG certification is required
EMR: eCW
