Case Manager Home Supervision/Home Incarceration

We believe every child is a person of worth, entitled to God’s gift of wholeness of life.Classification: Contract, paid monthly after submission of required billing documentsLocation: Northern KYCompensation: $14 per diem for each active client per day, $0.32/mile reimbursement.Summary: Youth that are court-ordered to be on home incarceration or home supervision are processed byDepartment of Juvenile Justice/Detention Alternatives Coordinators (DAC).The DACs then make referrals to theKentucky United Methodist Children’s Homes case managers. The youth will be monitored by the Case Manager to ensure the completion of the court mandates - this monitoring could include GPS monitoring equipment placed on the youth and in the home. The Case Manager will monitor the youth weekly through face-to-face contacts and as DAC specifies. Additional daily contacts will be in the form of telephone calls to specified locations depending on the individual situation. Reports regarding the youth’s performance on this mandate will be provided weekly to the referring DAC.Job Specific Duties and ResponsibilitiesProvide contact with the youth, as identified by the DAC including but not limited to one face to face contact per week with daily telephone contactRemain available via cell phone for on-call responsibilities at all times, or communicate with Director of ATD Serviceswhen not availableReceive and process referral/intakes as required by funding sourceAssure proper knowledge of tracking equipment use and maintenanceAccountable for the proper documentation and timely completion of all paper work associated with the requirements of the position, including the collection of date for: monthly billings, weekly Progress Reports, budget and year-end program reportsAttends professional meetings, workshops, seminars, and other training sessions necessary to maintain professional expertiseCommunicates all pertinent information to the Director of ATD ServicesMaintain collaborative relationships with community workers and DAC for case planningKeep abreast of issues facing youth todayJob SpecificationsMust be at least 21 years old and have a high school diploma. Previous case management or juvenile services experience preferredMust have a valid Kentucky driver’s license and submit to an intensive background investigation.
