Join us in making a real difference in the health of our community.Position Title Community Outreach LiaisonPosition Summary Essen Health Care is seeking a motivated and community-focused Community Outreach Liaison to support outreach initiatives across the Bronx, Brooklyn, Queens, and surrounding communities. This role is responsible for building relationships within the community, promoting healthcare services, supporting patient engagement efforts, and increasing awareness of Essen Health Care programs and resources.The ideal candidate is personable, organized, culturally aware, and passionate about improving access to healthcare for underserved populations.ResponsibilitiesConduct community outreach activities to promote Essen Health Care services and programsBuild and maintain relationships with community organizations, local businesses, schools, senior centers, shelters, and other partnersAttend health fairs, community events, and outreach initiatives on behalf of Essen Health CareEducate community members on available healthcare services, insurance resources, and wellness programsAssist patients with scheduling appointments and connecting to appropriate departmentsSupport membership growth and patient retention initiativesMaintain outreach logs, event tracking, and community engagement reportsDistribute marketing and educational materials within the communityCollaborate closely with operations, marketing, care management, and clinical teamsRepresent Essen Health Care professionally and positively in all community interactionsQualificationsMinimum High School Diploma or GED requiredAssociate’s or Bachelor’s degree preferredPrevious experience in community outreach, customer service, healthcare, social services, or patient engagement preferredStrong interpersonal and communication skillsAbility to work independently and in a team environmentBilingual skills strongly preferred (Spanish, Russian, Chinese, or other languages a plus)Comfortable working in the field and traveling locally throughout NYC boroughsBasic computer skills including Microsoft Office and email communicationPreferred SkillsKnowledge of NYC communities and healthcare resourcesStrong relationship-building and networking abilitiesCompassionate and patient-centered approachAbility to multitask in a fast-paced environmentEssen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population
