Opportunity Highlights Schedule: M-F, Full-timeTypes of Cases: Geriatric care for frail elderly population age 55 or older with chronic care needs; conduct history and physical on new participants, event-based and periodic reassessments, develop medical treatment plans, supervise interdisciplinary team care, chronic disease managementCredentialing: Self-Query NPDB report required at time of presentation (must be pulled within same month as submission). Trinity Health does not reimburse for any application or credentialing fees.
