In this position, you won't just be "working a floor"—you will be managing the continuum of care for high-risk geriatric patients. This is a permanent, full-time position in a collaborative outpatient setting.Key Responsibilities:Care Transitions: Be the primary point of contact for patients transitioning from hospital to home.Clinical Triage: Perform patient assessments, manage telephone inquiries, and coordinate care across various services.Direct Nursing: Administer immunizations, perform lab work, and assist with clinical procedures.Education: Empower patients and their families by providing clear instruction on medications and ongoing treatment plans.Qualifications:Active WA State LPN License.1+ year of clinical nursing experience.Experience with Geriatrics, PACE, or Outpatient clinics is highly preferred.Proficiency in Epic or similar EHR systems.Strong critical thinking skills and the ability to prioritize in a fast-paced environment.Schedule:Stable Monday–Friday schedule (Core hours 7:30 AM – 6:30 PM).
