All while understanding that together we are even better.Job Description Summary:Responsible for implementing the Readmission Reduction Program targeting patients readmitted within 30 days of discharge. This position collaborates with an interdisciplinary team to provide transitional management for high-risk patients. Additionally, this role oversees improvements related to publicly reported data and quality initiatives. In addition, this position assists in acquiring and maintaining various Disease Specific Certifications. Collaborates with the Patient Experience Manager to enhance patient satisfaction in the transition of care domain.Job Description:Ensures compliance with Disease Specific standards of care and other regulatory bodies.Participates in conferences and teleconferences for Readmission Reduction programs. Compiles data for audits and accreditation.Coordinates patient care post-discharge for high-risk readmission patients.Stays updated on regulatory requirements. Engages in discussions to reduce readmissions. Gathers and reports data accurately for audits.Manages follow-up care to prevent readmissions. Develops and implements strategies to reduce readmissions.Knowledge, Skills, and Abilities:Demonstrated proficiency in Acute Care, knowledge, and skills. [Required]Knowledge of the information and techniques needed to diagnose and treat patients. This includes symptoms, treatment alternatives, drug properties and interactions, and preventative healthcare measures. [Required]Knowledge of principles and processes for providing patient care. This includes patient needs assessment, meeting quality standards for services, and evaluation of patient satisfaction. [Required]Ability to lead, support and build on current efforts of the readmission reduction program. [Required]Demonstrates initiative, flexibility, integrity and diplomacy.
