Department: EndocrinologySchedule: Full TimeHospital: Witham Health ServicesLocation: Lebanon, INJob Summary:Assist in planning, organizing and implementing diabetes care plan under the guidance of endocrinologist and other healthcare professionals. Train patients in the correct use of prescribed therapies to successfully manage their current disease state.Possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs. Communicates regularly with other healthcare professionals involved in patient care. Records pertinent information of patient's diet, current nutritional status, recommendations and plan of care in the medical record for physician and other healthcare professional's information. Reviews and develops new educational materials and nutrition education programs to be used in patient and employee education. Collaborates with physician in the planning and implementation of diabetic education and treatment.Minimum Requirements:The position requires registration or registration eligible status by the Commission Dietetic Registration (CDR).Requires continuing education as specified by the Commission Dietetic Registration (CDR) for registration as a dietitian/nutritionist.Insulin pump and CGM experience preferred.Requires certification (licensure) eligibility status by the State of Indiana at hire or will complete within 2 years of date of hire.Graduate of an accredited college and registered by the Commission Dietetic Registration (CDR).Prior patient education experience preferred.Equal Opportunity Employer
