SUBSTITUTE SCHOOL NURSEPRIMARY PURPOSE:To provide health related services to students at assigned school(s) and assist with providing instruction for teachers and staff concerning various medical conditions/needs of students.MINIMUM QUALIFICATIONS:Certification/LicenseLicensed Practical NurseProficient SkillsWritten and Oral CommunicationPlanningOrganizationDESCRIPTION OF DUTIES:Accessible to students and staff when questions or concerns occur.Assist in hearing/vision/dental/nutrition screenings and follow-up as needed.Administration of medication to students.Coordinate with County School Nurse to participate in kindergarten registration.If assigned to a middle school, assist County School Nurse in the scoliosisscreening of middle school students.Assist County School Nurse in coordinating health fairs for students and/or faculty as requested.Assist County School Nurse in providing appropriate training to teachers andstaff in universal precautions, medical conditions and delegated medical procedures.Maintains records and reports for all students who have a health relatedcondition and provide assistance to students and/or parents as needed.Maintain a daily clinic log of student visits.Ensure that all schools use proper forms, permission slips, and schedules fordispensing medication.Assist County School Nurse in the annual review of immunization records ofall students in pre-kindergarten, kindergarten, and sixth grades.Performs other duties as assigned by appropriate administrator.
