This role serves as a liaison between clinicians, patients, families, and office staff, supporting care planning, documentation, compliance, and day-to-day operations.Reports to: Executive DirectorClinical CoordinationAssist clinicians (RNs, LPNs, therapists, hospice staff) with care coordination and schedulingSupport intake and referral processes, including gathering patient information and verifying eligibilityTrack and communicate patient care needs, updates, and changes in conditionCoordinate interdisciplinary team (IDT) meetings and documentationDocumentation & ComplianceReview clinical documentation for completeness, accuracy, and regulatory complianceEnsure timely submission of visit notes, plans of care, and physician ordersAssist with electronic medical record (EMR) data entry, order tracking and maintenanceSupport compliance with CMS, DPH, CHAP regulations Patient & Family SupportServe as a point of contact for patients and families regarding services and schedulingProvide basic education on home health or hospice services (non-clinical guidance)Facilitate communication between care teams, patients and families Operational SupportManage clinician schedules and optimize visit routesAssist with supply coordination and equipment trackingSupport quality improvement initiatives and audits QualificationsEducation & ExperienceLPN, associate or bachelor's degree in nursingExperience in home health, hospice, or healthcare administration strongly preferredFamiliarity with EMR systems (e.g., Matrix Care, Homecare Homebase, Epic)Skills & CompetenciesStrong organizational and multitasking skillsKnowledge of home health and hospice workflows and regulationsExcellent communication and interpersonal skillsAttention to detail and ability to manage deadlinesAbility to work collaboratively in a fast-paced, interdisciplinary environment Working ConditionsOffice-based with frequent interaction with clinical staffMay require occasional on-call support or flexible schedulingHigh attention to detail and deadlines
