You can be a part of our vision to be a leader in the integration of physical, mental and social health to make a real difference in the lives of others!We are currently seeking a full-time, day-shift Skill Builder II to join our Children's Program, located in Jay and Blackford County, Indiana.About the Role of a Skill Builder II:As a Skill Builder II, youll play a vital role in helping patients and families develop essential life skills that support developmental milestones and reduce psychosocial barriers. Youll work closely with therapists and medical staff to coordinate care and implement individualized treatment plans.Skill Builder II Key Responsibilities:Collaborate with therapists to develop, implement, and revise treatment plans.Deliver individual and group skills training for adults and children, focusing on:Self-care and daily life managementInterpersonal communication and parentingIllness and anger managementPositive coping and problem-solving skillsProvide case management services aligned with treatment goals.Support integrated care by coordinating with medical providers and identifying health-related needs.Complete CANS/ANSA reassessments and maintain accurate clinical documentation.Serve as a liaison between patients and external agencies or institutions.Participate in ongoing professional development and training.What Were Looking For in a Skill Builder II:Were seeking compassionate, motivated individuals who are passionate about whole-person care and thrive in a collaborative, team-based environment.
