Social Worker
Job Summary: The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are essential.Location:Burbank, California, United StatesResponsibilities:- Provide skilled nursing care to patients.- Perform patient assessments and monitor their condition.- Administer medications and treatments as prescribed.- Develop and implement patient care plans.- Collaborate with the interdisciplinary team to ensure comprehensive patient care.- Communicate effectively with patients, families, and healthcare providers.- Perform Care Coordination/Discharge Planning.Required Skills & Certifications:- Active Registered Nurse license in the state of working (CA state license required).- Clinical Assessment, Medication Administration, and Treatment Planning skills.- Minimum of 2 years of acute medical Care Manager experience performing Care Coordination/Discharge Planning.- Interdisciplinary Team Collaboration skills.- Excellent communication and interpersonal skills.- Ability to work effectively in a fast-paced environment.- Bachelor's degree in Nursing or related field.- Driver's license, car insurance, and automobile for field work.Preferred Skills & Certifications:- Specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room.Special Considerations:- First-time travelers are accepted but must be screened.- Weekend requirement: typically one full weekend per 4 weeks, subject to department need.- No floating expectation.- RTO Restrictions: No more than 7 days.Scheduling:- Start date: ASAP.- Days shift.
