Cooley Dickinson's VNA & Hospital is also a vital part of our network, providing home-based services through-out Hampshire and Franklin counties.Job SummaryFull time 40 hours- DaysThe VNA RN Case Manager is responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
