In this role, you will provide assessment and evaluation of clinical operations for assigned facilities, ensuring optimal outcomes, regulatory compliance, and shared accountability. You will work closely with Regional Directors and facility leadership to identify trends, implement solutions, and promote a culture of continuous improvement—coaching and mentoring clinical leaders while helping shape the future of the organization.Key ResponsibilitiesLeadershipModel clinical confidence and accountability across all levels of the organizationIdentify clinical development opportunities for regional clinicians and facility personnelApply problem-solving and root cause analysis skills to resolve complex clinical concernsArticulate and champion the clinical vision and standards of excellence Support and facilitate Peaks Healthcare core values, practice standards, and code of conduct Establish priorities to leverage resources proactively and avoid crisis situationsStaff Training and DevelopmentIdentify facility strengths and clinical development needs; mentor and develop co-worker and facility clinical leadership in staffing, survey readiness, quality management, and customer serviceCollaborate with interdisciplinary teams to identify educational needs and communicate them to appropriate departmentsParticipate in training programs to meet organizational needs and ensure training consistencyClinical OperationsUse standard company reports and tools to monitor contract labor, open positions, supply costs, PPD, and other indicators to analyze trends and meet budget expectationsMonitor staffing ratios to validate compliance with company, state, and federal requirementsCollaborate with Clinical Reimbursement to ensure documentation adequacy for medical necessity and reimbursement complianceTrain and direct leadership on survey process management; ensure facility team competency with survey management and implement corrective plans when weaknesses are identifiedReview the Statement of Deficiencies (2567) to determine opportunities for Informal Dispute Resolution (IDR) and facilitate plan of correction completion to meet deadlinesOversee Risk Management reporting, ensuring high-risk events and reportable occurrences are communicated and submitted in accordance with State and Federal requirementsPartner with the Clinical Director of Pharmacy Services on pharmacy initiatives; maintain current knowledge of regulations and communicate changes to the appropriate leadersContinuous Quality ImprovementCommunicate regularly with the Leadership Team regarding facility and regional quality and operational concernsIdentify trends and collaborate to implement solutions; monitor effectiveness and sustain improvementValidate facility site visits are completed, analyze results, conduct root cause analysis, and provide timely feedback with recommended changesUtilize quality management reports (QIQM, Event Management, and other tools) to proactively evaluate and manage operational systemsMonitor and validate survey trends to manage resident care delivery and sustain substantial complianceQualificationsRegistered Nurse (RN) license required5–8 years of clinical experience in Long Term Care/Skilled Nursing FacilitiesWorking knowledge of long-term care clinical and operational systems, including reimbursement regulations, nursing practices, and federal/state regulatory requirementsStrong analytical skills with the ability to understand financial systems and budgetary managementAbility to travel by car and plane, overnight as needed; frequent travel throughout the State of New Mexico expectedBenefitsCompetitive pay401KHealth, dental, and vision insurancePaid Time Off (PTO)Supportive and collaborative work environmentGrow With UsAt Peaks Healthcare, we are committed to developing our team.
