Your work will help patients navigate the healthcare system and achieve better long-term outcomes.Key responsibilities include:Coordinating and monitoring patient care to optimize health outcomesIdentifying and supporting high-risk patientsDeveloping and managing individualized, patient-centered care plansProviding patient education and motivational interviewing to promote self-managementCollaborating with primary care providers, specialists, hospitals, and community resourcesConducting post-hospital follow-ups, medication reconciliation, and care transitionsManaging a caseload (up to 250 patients) with a focus on prevention and continuity of careMaintaining accurate documentation and supporting care management billingWork EnvironmentClinic/office-based settingOccasional travel between clinic locationsPhysical requirements include standing, walking, and lifting up to 25 lbsNon-exempt position; occasional hours over 40/week may be requiredAbout UGLFHCUpper Great Lakes Family Health Center is committed to providing high-quality, compassionate care to the communities we serve.
