Home Care Community Liaison and Outreach Specialist

The Community Liaison / Outreach Coordinator is responsible for driving census growth through strategic relationship-building, community engagement, referral development, and social media marketing. This role blends in-person outreach with digital engagement to expand brand visibility and increase client referrals. In this vital role, you will serve as a bridge between healthcare providers, community organizations, and the populations we serve. Your energetic approach will help expand outreach efforts, foster strong relationships, and promote access to essential home care services.ResponsibilitiesDevelop and implement outreach strategies to connect with underserved populations and promote home care services effectively.Build and maintain collaborative relationships with community organizations, healthcare providers, social service agencies, and local leaders.Develop and maintain referral relationships with hospitals, rehabilitation centers, skilled nursing facilities, physician offices, case managers, discharge planners, and social workersConduct in-person visits to healthcare facilities and community organizations to present agency servicesRepresent the agency at health fairs, senior expos, networking events, and community programsBuild relationships with assisted living facilities, senior centers, churches, and community leadersEducate referral sources about agency services, eligibility, and the intake processTrack referral sources and maintain consistent follow-up communicationIdentify and pursue new referral opportunitiesMonitor referral trends and identify growth opportunitiesSupport branding initiatives to strengthen community presenceManage agency social media platforms (Facebook, Instagram, LinkedIn, etc.)Respond to online inquiries and messages professionally and promptlyCoordinate digital campaigns to promote hiring events and service offeringsDistribute marketing materials to referral partnersAssist with the creation of flyers, brochures, and promotional materialsMaintain updated contact lists for community partnersStrengthen brand visibility within the service areaSkillsStrong interpersonal and relationship-building skillsFamiliarity with home care industry (preferred but not required)Experience in healthcare marketing, home care, hospice, senior services, or related fieldExisting community connections in the local healthcare market (strongly preferred)Energetic and outgoing personalityResults-drivenComfortable networking independentlyPassion for serving seniors and vulnerable populationsSales-oriented mindset with ability to close referral opportunitiesSelf-motivated and goal-drivenStrong organizational and follow-up skillsStrong public speaking skills with the ability to engage diverse audiences confidently.Experience managing business social media accountsBasic knowledge of social media analyticsContent creation and branding awarenessCompany DescriptionPleasant Vine Home Carewww.pleasantvinehc.com610-226-2121Company DescriptionPleasant Vine Home Care\r\nwww.pleasantvinehc.com\r\n610-226-2121
