LPN Charge Nurse Assisted Living

Promenade at University Place - Summary: The LPN is responsible for the supporting the wellness department manager with staffing matters, case management, and medication administration/supervision of the residents, including staff personal care services in the Assisted Living Community.. Responsibilities include, but not limited to accurate transcribing orders, working with residents doctors and pharmacies to ensure the MAR's are up to date at all times, medication ordering and procurement, hiring and training of the med techs and personal care aids; annual review and ongoing coaching and counseling of the staff members: completion of case management notes, staffing schedule and submitting payroll hours. Ensure that the universal worker concept is implemented.This is a hands-on position where assisting residents with activities of daily living, medication administration and acting as the charge nurse on the resident floors will be required. As the leader of the resident care services is expected to develop and maintain a positive atmosphere for residents, families, visitors and staff.ResponsibilitiesEnsure resident ADL services are executed in line with Plan of Care;Ensure that ADL activity is properly documented, approved and provided to corporate for billingAccurate transcribing orders, working with resident doctors and pharmacies to ensure the MAR's are up to date at all timesDispense medication to residents as per the Medication Administration RecordMedication ordering and procurementSupervision of the med techs and personal care aids;Ensure staffing levels and scheduling are in line with resident census levelsUpdating case management notes;Coordinate care with residents, family members and physiciansAs the leader of the resident care services is expected to develop and maintain a positive atmosphere for residents, families, visitors and staffCover staffing shortages as neededEnsure staff is properly trained and certifiedOn-call 24/7 hours for Emergencies and Manager on Duty responsibilities.Other duties as may be required or necessaryQualifications:LPN with One (1) year experience in long term care or assisted livingCurrent state license as an LPN in New YorkAbility to handle multiple prioritiesAble to supervise and work part of a teamExcellent verbal and written communication skillsHas good organizational and time management skillsExperience with seniors preferredAble to make responsible choices and decisions and act in the resident's best interestAbility to lift 25-35 pounds
